Mory, reading your website I see that you have your installers (Site Technicians?) get the client to fill in a survey after each fit. May I ask which questions you find offer useful feedback? Does anyone else do this and if so what are the useful questions?

Excellent question. If I may add, do you give them any incentives for filling it out?

I would think that the data would be more accurate if asked a week or so after the install, as the bloom comes off the rose.

We have been doing surveys for several years now and I feel it is a valuable tool. I will find out about things that I would have never found out if not for the surveys. I feel that they should be left with the client so they can fill them out at their leisure, if they have an installer there they may not be completely honest.
